Medication Administration

In keeping with Board of Education Policy 5330, Administration of Medication, and following guidance established by the Department of Public Instruction, all medication that is administered to students or taken by a student while at school requires a permission form, including over-the-counter medications. If the medication is a prescription, a physician's signature is required. Parents, please download the following form, complete it, and return it to the school office. The form should be completed for both prescription and over-the-counter medications.
